If you have searched for custom hospital scheduling software, you have met a wall of development agencies. That is not a criticism of them, and some do good work. It is a structural point about the answers you are getting: the question “should we build this?” is being answered almost entirely by companies whose revenue depends on yes.
This post is the other side. We built a nurse scheduler, we run it, and we sell it as a service rather than as software you operate. That gives us a bias too, and the honest way to handle a bias is to name it and then argue against yourself where the argument holds. So the section on when building makes sense is written straight, with no hedging back toward us.
Who Actually Answers This Question Online?
Mostly development companies.
Searching in September 2026 for custom hospital staffing software development, 14 of the first 19 organic results were build-side pages from development firms: OSP Labs, Empeek, TATEEDA, SolGuruz, HQSoftware, Arkenea, Thinkitive and Computools among them. Their pages are well made and their capability is real. The point is not quality, it is incentive.
The remaining five are worth naming, because they are the balance that does exist: a couple of product round-ups, a vendor blog, a YouTube review, and a Reddit thread where somebody simply asks which software to use. So the buy side is present. It is outnumbered roughly three to one, and none of it is aimed at a hospital your size.
That matters for one reason. On a question with two answers, you are reading a shelf where one answer has a marketing budget and the other does not. Almost nobody is paid to write “do not build this”, so it rarely gets written, which leaves a small hospital reading a lopsided literature and reasonably mistaking it for the whole picture.
The second thing missing from that shelf is your size. Those pages are written for staffing agencies, hospital networks and multi-specialty groups. A 25-bed hospital with one nurse manager who also takes clinical shifts is not the buyer they have in mind, and the advice does not shrink cleanly.
What Does Building Really Involve?
Two commitments, and the first one gets all the attention.
The build. A defined project with a scope, a price and an end date. This is the part agencies quote for and the part hospitals plan around. It is the smaller commitment.
The ownership. After launch, you own it. Not the agency, unless you keep paying them. That means every one of these lands on your hospital permanently:
- Changes, and mostly not the ones you expect. Vendors like to warn that regulations shift under you. Being straight about it: the federal rules that bind a critical access hospital schedule have been stable, and the recent movement mostly has not touched them. Texas renumbered its hospital licensing rules in January 2025, which changed the citation and not the duty. The Joint Commission reorganized staffing into a named performance goal effective January 2026 without adding requirements. A federal minimum staffing standard was repealed in February 2026, and it applied to long-term care rather than to your hospital. The recurring work is more mundane than regulation and that is exactly why it gets left out of the quote: you open a swing bed, you change your night shift pattern, a union agreement adds a rule, you hire your first LPNs and now skill mix matters. Each one is a change request to whoever holds the code.
- The person who knows it. Custom software has a keeper. When that developer moves on, the knowledge goes with them, and the next person starts by reading unfamiliar code before they can safely touch a system your schedule depends on.
- Everything unglamorous. Backups, security patching, uptime, and someone reachable when it breaks at 5 a.m. on a Sunday, which is exactly when the night charge nurse needs to see who is coming in.
None of that is an argument that building is wrong. It is an argument that the quote you were given covers the first commitment and not the second, and the second one never ends.
What Makes a Scheduler Harder Than It Looks?
The scheduling is the easy half. The rules are the hard half.
Building a grid that assigns names to shifts is a solved problem, and a competent developer will demo one quickly. What takes the time is everything that decides whether an assignment is allowed:
- Who has to be on duty. 42 CFR 485.631(a)(5) requires that “a registered nurse, clinical nurse specialist, or licensed practical nurse is on duty whenever the CAH has one or more inpatients.”
- Who has to assign and supervise the care. That is a different section. 42 CFR 485.635(d) requires a registered nurse to provide or assign the nursing care of each patient, and requires a registered nurse, or a physician assistant where State law permits, to supervise and evaluate it.
- State rules on top. In Texas, 26 TAC 505.41(o)(2)(D) requires an RN on duty in each building of a licensed hospital containing at least one nursing unit where patients are present. So a night that satisfies the federal rule with a licensed practical nurse alone does not satisfy Texas. A scheduler that only knows the federal rule will happily produce that night and tell you it is fine. Confirm how this applies to you with your compliance lead, since licensing and building layout both affect it, and remember there are two inspections: a CMS survey against the federal conditions, and a separate state licensing survey.
- Fairness that lasts longer than one schedule. Anyone can spread weekends evenly inside a single four-week schedule. Stopping the same three people carrying every weekend across the year means each new schedule has to know what the previous ones did. That is a design decision, and it is painful to add afterwards.
- What happens when there is no answer. The interesting behavior of a scheduler is not filling a shift. It is what it does when the shift cannot be filled: whether it leaves a gap loudly, quietly, or by breaking a rule it should not break.
- Talking to your other systems. This is the one that overruns budgets. A schedule has to reach timekeeping and payroll somehow, and if the answer is a real interface into your EMR rather than a file export, you have added the hardest part of the project. Decide early whether you are building an interface or moving spreadsheets, because those are different projects with different price tags.
That last one is where home-built schedulers usually disappoint. They are built and tested against the normal week, and the value of a scheduler is almost entirely in the abnormal one.
When Does Building Genuinely Make Sense?
Four situations, stated plainly. If you are in one of them, build, and this post is not for you.
1. Your scheduling problem is genuinely unusual. Not “we do it our way”, which every hospital says, but structurally unusual: an unusual mix of services sharing one roster, a union agreement with rules no product supports, a research or teaching obligation that shapes assignments. Products encode the common case. If you are not the common case, you will fight a product forever, and a custom build stops that fight.
2. You already have a software team and they will still be here in five years. Not a contractor, not one talented person in IT. A team, with a manager, a budget line and a succession plan. If you have that, the ownership commitment above is already a thing you do well, and building is a reasonable use of it.
3. Scheduling is strategically yours. Some health systems consider workforce deployment a competitive capability and want it in-house on principle. That is a legitimate strategy, and if leadership genuinely holds it, buying will always feel like a compromise.
4. The money you have is capital, not operating budget. Reasons 1 to 3 describe larger organizations. This one does not, and it argues against buying from anyone, us included. A build is a capital project. It can be depreciated, it can sit inside a grant, and it ends. A subscription is an operating line that never ends and that your successor inherits. If you are holding restricted funds that can be spent on a system but not on a monthly bill, the comparison is not total cost of ownership at all, and a five-year spreadsheet will not capture why. The reason that does not hold up on its own: “it will be cheaper.” It can be cheaper to build. It is rarely cheaper to own. Unless reason 4 applies, run the five-year numbers before committing rather than after.
If none of the four applies, a 25-bed hospital is buying something that already exists.

Why Did We Build One and Not Sell It as Software?
Because at this size the software was never the hard part, and pretending otherwise would have sold the wrong thing.

The reason is what we saw in more than 30 conversations with nurse managers before we built anything. Those were our own conversations rather than a study, and what came out of them was that the problem was rarely a missing tool. The schedule needed someone’s attention every week, and that person was already doing two other jobs. A better tool hands them the same work with a shorter learning curve in front of it.

Four limitations, and these are the ones an administrator asks about rather than the ones a vendor volunteers.
We do not integrate with your other systems. No Epic, Cerner, Meditech or athenahealth connection, no HL7 or FHIR interface, and no payroll or HRIS integration. Rosters come in from Excel and schedules go out as files. If your IT lead’s first question is how this talks to the EMR, the answer is that it does not, and that is a real reason to choose something else. It is also the question to put to every vendor and every developer you talk to, because integration is where scheduling projects usually overrun.
Your data leaves with you. Roster, schedules and the full change log export as spreadsheet files whenever you ask, during the relationship and at the end of it. Ask any vendor the same question and get the answer in writing, because a scheduling history you cannot take with you is a switching cost that does not appear on any price list.
Above 40 nurses there is no published price. Our flat tiers stop there, and larger rosters or multi-facility groups are a conversation rather than a number. If you counted your PRN and per-diem staff and went past 40, we are not being transparent with you, we are quoting.

Our Take
Most build decisions at small hospitals are not really decisions about software. They start with a bad experience of a product that did not fit, and building feels like the way to be understood. That instinct is sound and the conclusion usually is not, because the thing that did not fit was rarely the code. It was that nobody had time to configure it properly, keep it current, and sit with it every cycle. Building produces a system with exactly the same requirement, and now you own the maintenance too. Before writing a specification, work out who was going to look after the thing you already had. If the answer is nobody, that is the problem to solve, and a new build will inherit it intact.
What to Do This Week
- Name the person who will own it in year three. Not the person building it. If you can answer that in the hallway and the answer is nobody, the build conversation is finished and the rest of this list is about buying instead. This is the fastest question on the list and it decides the most.
- Ask your DON to write down the three rules the schedule must never break. If nobody can name them in a meeting, no developer can build them, and a specification resting on “the normal rules” is how a scheduler ends up producing a night that fails your state requirement.
- Settle whether this is capital or operating money before you compare anything. If restricted or grant funds are in play and cannot cover a subscription, that shapes the decision more than any five-year total, and it is worth knowing before your CFO builds a spreadsheet that cannot represent it.
- Get one build quote and one subscription quote, then ask the same two questions of both. What happens when a rule or a unit changes, and what happens if the person who knows this leaves. You do not need a market survey. You need two real numbers and two honest answers, and the answers matter more than the numbers.
